















Care Center
Attached to Lincoln Health is 22 bed, Licensed Care Center offering comfortable, compassionate, and homelike long term care. The Care Center serves families in Lincoln County and the surrounding eastern plains communities. The Care Center is proud to have been awarded 'America's Top Nursing Home' Award in 2009 and 2013. Patients of the care center have access to the following services:
Activities: Staff interacts through one-to-one or group activities, to support the Residents' physical, mental, emotional, psycho-social and spiritual needs to improve the quality of life.
Therapy: Skilled services are offered by licensed therapists in physical, occupational, and speech therapies. Services are provided based on the Residents' needs for mobility, positioning, cognition and swallowing.
Restorative: Restorative nursing programs are provided for walking, dining, bowel/bladder, activities of daily living, range of motion, communication, transfers, and bed mobility.
Social Services: Staff provides communication with families and residents about the admission process, orientation to the facility, discharge planning, social needs, coordination of Medicaid benefits, and making resource connections.
Behavioral Health: Behavioral health services are provided to ensure the residents maintain the highest practical physical, mental, and psycho-social well-being.
Nursing Services
Activities of daily living (ADL's) are the things you do every day from the time you get up, until the time you get back to bed. We provide the care needed for the residents to be able to meet those daily needs. Evaluated on an individual basis, these can include personal care needs such as toileting, dressing, and grooming, to eating and hydration assistance. Mobility needs are aided through individualized strengthening and exercising, along with any pain, wound care, respiratory or medication needs. We serve the whole person, addressing behavior, communication, visual, and psycho-social needs to transportation for appointments. We understand the challenges of transitioning to a new living arrangement and aid in the adjustment with contact, care, and compassion.
